    A Practical Example for Model-Driven Web Requirements

    The number of approaches for Web environments has grown very fast in the last years: HDM, OOHDM, and WSDM were among the first, and now a large number can be found in the literature. With the definition of MDA (Model- Driven Architecture) and the acceptance of MDE (Model-Driven Engineering) techniques in this environment, some groups are working in the use of metamodels and transformations to make their approaches more powerful. UWE (UMLBased Web Engineering) or OOWS (Object-Oriented Web Solutions) are only some examples. However, there are few real experiences with Web Engineering in the enterprise environment, and very few real applications of metamodels and MDE techniques. In this chapter the practical experience of a Web Engineering approach, NDT, in a big project developed in Andalusia is presented. Besides, it shows the usability of metamodels in real environments

    Late Holocene Aleppo pine (Pinus halepensis Miller) woodlands in Mallorca (Balearic Islands, Western Mediterranean): Investigation of their distribution and the role of human management based on anthracological, dendro-anthracological and archaeopalynological data

    The pioneering nature of Mediterranean pines and their phytosociological role have been largely discussed in relation to different agents (e.g., edaphic, climatic or anthropogenic). In this context, Aleppo pine is one of the most widespread pine species in the Mediterranean basin, as it is especially adapted to climatic constraints, such as drought and high seasonality, and has a high tolerance for salinity and strong coastal winds. It is also well adapted to regeneration after anthropogenic landscape disturbances, highlighting its important after-fire regeneration rates. In this sense, phytosociological studies conducted in Mediterranean landscapes have found that this species'' wide distribution is mostly due to its rapid regeneration after human landscape transformation, including fire, and the abandonment of agricultural lands. Aleppo pine is considered to broadly develop after human action in sclerophyllous formation, in which it would be scarce or absent without human intervention. Parallel, paleoenvironmental and archaeobotanical studies have attempted to trace these trends back to prehistoric times to investigate this species'' role in Late Pleistocene and Holocene vegetation and evaluate the role of climate and human action in its diachronic dynamics. In this study, we present a compendium of anthracological, dendro-anthracological and archaeopalyonological data with the objective of (i) investigating the nature and distribution of Aleppo pine on the island of Mallorca and (ii) evaluating the possibility that human action could have resulted in the spread of this pine species during the first two millennia of permanent human occupation of the island (c. 2300 cal. BCE–1st-century ACE). Investigating these archaeobotanical datasets, as well as making comparisons with anthracological and paleoenvironmental studies in neighbouring Mediterranean zones (Iberia), allowed us to attest that Aleppo pine is a natural, pre-human component of the Holocene vegetation of the island, and it is especially well-adapted to coastal environments. Moreover, we describe the trends and characteristics of the human management of pine woodlands through anthracology and dendro-anthracology, suggesting that human action did not provoke widespread growth of Aleppo pine in Mallorca at the expense of other vegetation types during prehistory. Such processes, well-documented by current phytosociological studies, probably began at some unknown point after the Romanisation of the island

    Burnout syndrome and job satisfaction in spanish and argentinean health workers

    Se realizó un estudio transversal sobre Burnout y satisfacción laboral a 107 profesionales del ámbito sanitario hispanohablantes de España y Argentina. Médicos, enfermeros y auxiliares respondieron al cuestionario MBI (Maslach Burnout Inventory) y al Inventario de Satisfacción Laboral (ISL *66). Los objetivos del estudio fueron, por una parte, investigar la prevalencia de Burnout en España y Argentina, así como también, comprobar si existe relación entre el Burnout y la Satisfacción Laboral. Se halló una mayor prevalencia de Burnout en el personal sanitario español frente al argentino, y no se encontró correlación entre Burnout y satisfacción laboral en el personal sanitario de ambos países. Una posible explicación de estos resultados se encuentra en la coyuntura macroeconómica de los países en los últimos años.A cross-sectional study of burnout and job satisfaction for 107 health professionals of the speaking area of Spain and Argentina was conducted. Doctors, nurses and auxiliary answered two questionnaires: the MBI (Maslach Burnout Inventory) and job satisfaction inventory (ISL * 66). The aims of the study were to investigate the prevalence of Burnout in Spain and Argentina and to study if there is a relation between Burnout and Job Satisfaction. It was found more prevalence of Burnout in Spanish health professionals than in Argentinean and it was not found any correlation between Burnout and Job Satisfaction in both countries.     Use of rivaroxaban and acetylsalicylic acid as a combined treatment for peripheral arterial disease in Central Military Hospital

    Background: The objective of this research was to evaluate the behavior of 3 risk indicators for peripheral arterial disease in patients under oral treatment with rivaroxaban 2.5 mg every 12 hours plus, acetylsalicylic acid 100 mg every 24 hours. It was hypothesized that the oral combination of rivaroxaban and acetylsalicylic acid presents a therapeutic advantage over other treatments.Methods: A prospective longitudinal and non-randomized study of a single center was performed. 59 patients with peripheral arterial disease were included and treated with acetylsalicylic acid + rivaroxaban. Peak systolic velocity, ankle-brachial index and C reactive protein index were evaluated.Results: Significant changes were found at month 1 and 3 of follow-up in maximum systolic velocity, ankle-arm index and C-reactive protein index. The baseline peak systolic velocity (PSV) in the anterior tibial artery had significant differences after one month of treatment (p=0.001) and after 3 months (p=0.001). The baseline PSV in the posterior tibial artery had significant differences compared to the values found at the month of treatment (p=0.001) and 3 months (p=0.001). In the ankle-brachial index a baseline median of 0.790 was found, one month after the treatment of 0.795 (p=0.147) and 3 months after 0.800 (p=0.019). The mean baseline C-reactive protein obtained was 73.142 mg/l, at one month 87.233 mg/l (p=0.001) and at 3 months at 79.009 mg/l (p=0.294) with a standard deviation of 67.18, 74.78 and 69.69 respectively.Conclusions: The combined use of acetylsalicylic acid and rivaroxaban allows a clinical improvement in patients with peripheral arterial disease

    Management of a ruptured epidural catheter, an anesthesiologist's dilemma: a case report

    Epidural anesthesia is a widely used anesthetic technique in lower extremity surgeries although it is a relatively safe procedure, it can have complications, such as rupture of the epidural catheter. This is a 69-year-old male patient with a diagnosis of Wagner IV diabetic foot is presented, which was scheduled for left supracondylar amputation in which after epidural block, retention of the catheter tip in the epidural space at level L2-L3 was seen, so hemi laminectomy was performed in a second surgical stage in L2 and removal of the epidural catheter. Ideally a broken needle should be removed as soon as possible

    The epigenetic regulator Mll1 is required for Wnt-driven intestinal tumorigenesis and cancer stemness

    Wnt/β-catenin signaling is crucial for intestinal carcinogenesis and the maintenance of intestinal cancer stem cells. Here we identify the histone methyltransferase Mll1 as a regulator of Wnt-driven intestinal cancer. Mll1 is highly expressed in Lgr5(+) stem cells and human colon carcinomas with increased nuclear β-catenin. High levels of MLL1 are associated with poor survival of colon cancer patients. The genetic ablation of Mll1 in mice prevents Wnt/β-catenin-driven adenoma formation from Lgr5(+) intestinal stem cells. Ablation of Mll1 decreases the self-renewal of human colon cancer spheres and halts tumor growth of xenografts. Mll1 controls the expression of stem cell genes including the Wnt/β-catenin target gene Lgr5. Upon the loss of Mll1, histone methylation at the stem cell promoters switches from activating H3K4 tri-methylation to repressive H3K27 tri-methylation, indicating that Mll1 sustains stem cell gene expression by antagonizing gene silencing through polycomb repressive complex 2 (PRC2)-mediated H3K27 tri-methylation. Transcriptome profiling of Wnt-mutated intestinal tumor-initiating cells reveals that Mll1 regulates Gata4/6 transcription factors, known to sustain cancer stemness and to control goblet cell differentiation. Our results demonstrate that Mll1 is an essential epigenetic regulator of Wnt/β-catenin-induced intestinal tumorigenesis and cancer stemness

    International longitudinal registry of patients with atrial fibrillation and treated with rivaroxaban: RIVaroxaban Evaluation in Real life setting (RIVER)

    Background Real-world data on non-vitamin K oral anticoagulants (NOACs) are essential in determining whether evidence from randomised controlled clinical trials translate into meaningful clinical benefits for patients in everyday practice. RIVER (RIVaroxaban Evaluation in Real life setting) is an ongoing international, prospective registry of patients with newly diagnosed non-valvular atrial fibrillation (NVAF) and at least one investigator-determined risk factor for stroke who received rivaroxaban as an initial treatment for the prevention of thromboembolic stroke. The aim of this paper is to describe the design of the RIVER registry and baseline characteristics of patients with newly diagnosed NVAF who received rivaroxaban as an initial treatment. Methods and results Between January 2014 and June 2017, RIVER investigators recruited 5072 patients at 309 centres in 17 countries. The aim was to enroll consecutive patients at sites where rivaroxaban was already routinely prescribed for stroke prevention. Each patient is being followed up prospectively for a minimum of 2-years. The registry will capture data on the rate and nature of all thromboembolic events (stroke / systemic embolism), bleeding complications, all-cause mortality and other major cardiovascular events as they occur. Data quality is assured through a combination of remote electronic monitoring and onsite monitoring (including source data verification in 10% of cases). Patients were mostly enrolled by cardiologists (n = 3776, 74.6%), by internal medicine specialists 14.2% (n = 718) and by primary care/general practice physicians 8.2% (n = 417). The mean (SD) age of the population was 69.5 (11.0) years, 44.3% were women. Mean (SD) CHADS2 score was 1.9 (1.2) and CHA2DS2-VASc scores was 3.2 (1.6). Almost all patients (98.5%) were prescribed with once daily dose of rivaroxaban, most commonly 20 mg (76.5%) and 15 mg (20.0%) as their initial treatment; 17.9% of patients received concomitant antiplatelet therapy. Most patients enrolled in RIVER met the recommended threshold for AC therapy (86.6% for 2012 ESC Guidelines, and 79.8% of patients according to 2016 ESC Guidelines). Conclusions The RIVER prospective registry will expand our knowledge of how rivaroxaban is prescribed in everyday practice and whether evidence from clinical trials can be translated to the broader cross-section of patients in the real world

    P. falciparum In Vitro Killing Rates Allow to Discriminate between Different Antimalarial Mode-of-Action

    Chemotherapy is still the cornerstone for malaria control. Developing drugs against Plasmodium parasites and monitoring their efficacy requires methods to accurately determine the parasite killing rate in response to treatment. Commonly used techniques essentially measure metabolic activity as a proxy for parasite viability. However, these approaches are susceptible to artefacts, as viability and metabolism are two parameters that are coupled during the parasite life cycle but can be differentially affected in response to drug actions. Moreover, traditional techniques do not allow to measure the speed-of-action of compounds on parasite viability, which is an essential efficacy determinant. We present here a comprehensive methodology to measure in vitro the direct effect of antimalarial compounds over the parasite viability, which is based on limiting serial dilution of treated parasites and re-growth monitoring. This methodology allows to precisely determine the killing rate of antimalarial compounds, which can be quantified by the parasite reduction ratio and parasite clearance time, which are key mode-of-action parameters. Importantly, we demonstrate that this technique readily permits to determine compound killing activities that might be otherwise missed by traditional, metabolism-based techniques. The analysis of a large set of antimalarial drugs reveals that this viability-based assay allows to discriminate compounds based on their antimalarial mode-of-action. This approach has been adapted to perform medium throughput screening, facilitating the identification of fast-acting antimalarial compounds, which are crucially needed for the control and possibly the eradication of malaria

    The role of the Montjuïc promontory (Barcelona) in landscape change: human impact during roman times

    A multidisciplinary (historical, archaeomorphological, archaeological, palaeobotanical, and sedimentary) and multi-scale study was carried out to gain a better understanding of the long term landscape shaping of the Montjuïc promontory as a result of land use and palaeoenvironmental changes. Evidence suggests that, especially the southern slope of the promontory, was cultivated and used for grain storage during the Iberian Iron Age period. The presence of a Roman quarry, probably related to the foundation of Barcino, was also confirmed. Despite the prevalence of agriculture during the Roman period, pollen studies suggest that there were no significant changes in the plain of Barcelona.Peer Reviewe
